That you absented yourself from duty without leave at 7:50 on 23/7/12 contrary to No.7 of the Standing Orders for the Sanitary Watchman of the Hill District.

That you failed to go on duty until 5 a.m. on 29/7/17 contrary to No.7 of the Standing Orders.
You are hereby called upon to make such answer as you think necessary to the above charges showing on what you rely to exculpate yourself and to give such reasons as you can bring forward showing why you should not be dismissed by Wednesday the 14th instant.
